Lendable Kit Borrowing Agreement

Please note that lendable kits are only available to East Brunswick Public Library cardholders.

East Brunswick Public Library’s Lendable Kits are intended to promote curiosity, learning, and education among all members of the community.  

The kits may be borrowed for two (2) weeks and can be renewed up to 5 times.  

Overdue fines for kits are $5 per day, and users are responsible for all damaged or lost items when the kit is returned. Replacement cost of all kits are available on the library website or by calling the library. 

The signature below affirms that the user agrees to all library Circulation policies regarding kit use. 

Patron Information
Adults must fill out this information as well if they are planning to add children to this agreement.
Children under 18 Information
You will be able to add up to three children.

The signature below affirms that the Parent or Guardian accepts the responsibility of any kit checked out to the children listed above and agrees to all library Circulation policies regarding kit use.

As approved by the Library Board the lending policy for all kits is as follows: (9-19-17) 

"The library charges the East Brunswick cardholder the price listed in the library catalog or the current list price from the vendor the library uses for lost, damaged or unreturned kits. The library charges the cardholder the price of individual items in the kit if the individual item(s) is available from the library's vendor. The individual item prices are listed in the library catalog.  

A user agreement form must be signed before the first checkout of kits, and parents must sign the user agreement form in person, at the library, for their children under the age of 18."
